Job Description:

Provide comprehensive primary care across the lifespan in a team-based clinic. Duties include longitudinal panel care, same-day/urgent care, chronic disease management, preventive care, minor procedures (as competent), and EMR-based documentation/QI participation. Collaborate closely with nurses, pharmacists, behavioural consultants and other allied health to optimize access and outcomes.
