Description:

The C3 (Comprehensive Community Care) Program is a long-term maintenance and support program providing coordinated interdisciplinary care to frail older adults for the remaining duration of their life or until facility care placement.  This program is for clients living at home who require regular medical oversight and a day support program (rehab and recreational). Individuals admitted to the C3 program receive their primary medical care from the C3 physicians. 

The Family physician trained in Care of the Elderly medicine (COE) will provide patient centered care to clients in the program. The physician is part of a multidisciplinary team that provides comprehensive geriatric assessment, individual and group treatment, consultation, education, and home assessment services to support the client at home.

The COE physician will liaise with interdisciplinary team members to provide coordinated complex care.
They will provide medical services onsite at the medical clinic during the regular program and clinic hours, about 15- 20 hrs. per week.  They will also be available to clinic staff by phone between 8am - 5 pm on scheduled clinic days. The FTE ranges from 0.4 to 1.0.

The COE physician will participate in providing 24-hour, 7 day per week care by participating in on call shifts as assigned.  On call shifts are Monday to Thursday 1700hr- 0800hr, and weekends Friday from close of clinic to Monday at 0800hr. 


The successful candidate will also be required to apply for a Clinical Academic Appointment in the Department of Family Medicine, University of Calgary, if teaching learners in Seniors care. Physician will be responsible for a panel of clients as assigned by the medical site lead. This physician will be designated as the attending physician for this group of patients.

Qualifications:

    CFPC and Care of the Elderly designation 

    The candidate must have or be eligible for General Register with the College of Physicians and Surgeons of Alberta.

    This position is not sponsorship eligible.

    Must read, write and speak English.
