Description:

1.     Provide approximately 4 home visits per week. Home visits will involve comprehensive and routine patient assessments.
2.    Provide a combination of shared and longitudinal care within patient’s homes to delay or prevent admission to facility living, hospice or hospital. Shared care involves collaborative care with the attached primary care provider.
3.    Participate in weekly interdisciplinary team conference to review panel of patients, discuss new consults, and follow up visits.
4.    Provide afterhours support to patients within the program. This involves 24 hours 7 days a week coverage. Physicians in this program will participate in an on-call Rota to ensure continuous coverage for the patients enrolled in the program. On call shifts are Monday to Thursday 1700hr- 0800hr, and weekends Friday from after 1700hr to Monday at 0800hr.
5.    Work with the geographic Home Care teams to provide comprehensive care within the patient’s home.
6.    Work collaboratively with the physicians in the PCN within the assigned geographical location.
7.    Support transitions of care of complex older adults discharged from acute care, Complex Care Hub and Facility living.
8.    Attend family conferences when required.
9.    Be available to Home Care case manager and team members for telephone consultation.
10.    Provide educational and team development support to all team members.
11.    Work collaboratively with other care providers involved in the patient’s care, such as Mobile Integrated Care.
12.    Provide teaching for Family Medicine and Geriatric Medicine trainees

Qualifications:

 The successful candidate will hold a CCFP designation and be eligible for a Medical Staff appointment with Alberta Health Services. Clinical experience managing the care of Seniors Care patients will be beneficial. Completion of training in enhanced skills Care of the Elderly training program or Care of the Elderly Specialty designation is desirable. 


The candidate must be eligible for Licensure with the College of Physicians and Surgeons of Alberta.
Must read, write, and speak English.
