The Contractor shall provide services to the Calgary Department of Family Medicine as the Medical director of the Seniors Home Based Primary Care Program.

 

In the role of Medical Director of Seniors Home Based Primary Care, the service Contractor will primarily report through the Section Chief - Seniors Care to the Clinical Department Head of Family Medicine.  The Medical Director, Seniors Home Based Primary Care will have accountability to the Section Chief, Seniors Care, Department of Family Medicine for issues of credentialing, periodic reviews, and physician discipline.

The responsibilities of the Medical Director for Seniors Home Based Primary Care:

 

  1. Working with the Section Chief – Seniors Care and the Medical Director of Home Care to develop the strategic direction of the Seniors Home Based Primary Care program.

2.      Act as a liaison between the Seniors Home Based Program and the Department of Family Medicine to promote efficient community based medical care in the most appropriate venue.

4.      Develop admission criteria and policy and assure adherence within the program regions.

5.      Participate in the development of policy and guidelines for the program and communicate policies and changes to the medical staff to assure adherence within the program regions.

6.      Submit a yearly report to the Section- Chief, Seniors Care, Department of Family Medicine.

8.      Meet regularly with participating physicians within the program and promote inter- program communication and consistent practices and policies.

10.   Participate in recruitment and retention of physicians for Seniors Home Based Primary Care.

11.   Provide leadership and advice on QA and QI initiatives and act as liaison with the SGS cARP authorized representatives to meet key performance targets and contribute to the quarterly and annual SGS cARP reports.

12.   Complete periodic reviews of all physicians working in the Seniors Home Base Primary Care program in accordance to AHS bylaws.

13.   Develop and provide educational opportunities for the medical staff.

14.   Work with academic representatives from the Department of Family Medicine and other departments to develop student/resident rotations in the Seniors Home Based Primary Care Program.

The successful candidate will hold a CCFP/FRCP designation and be eligible for a Medical Staff appointment with Alberta Health Services. Clinical experience managing the care of Seniors Care patients will be beneficial, previous leadership experience is not required but will be an asset. Completion of training in enhanced skills Care of the Elderly training program, Care of the Elderly Specialty designation.

 

The candidate must be eligible for Licensure with the College of Physicians and Surgeons of Alberta.
